SB 577 Disclosure
This is a form describing my professional status, the status of homeopathy in
the state of California, the nature of the services I (Michael Pinkerton)
provide, my understanding of homeopathy, and my current level of education in
homeopathy.

I have a B.A. in Psychology from Oberlin College, a Doctor of Chiropractic from
Life West Chiropractic College, and I am a licensed chiropractor in the state of
California. The education that I received was in no way related to homeopathy,
and should not be construed as having any bearing on my status or ability
as a homeopath, nor on my education in homeopathy.

Homeopathy is alternative or complementary to healing arts services licensed by the state.

The service that I provide, homeopathic consultation, is not licensed by the state.

In one or more interview sessions, I will gather information about your sensations
and about your functioning in the world. Your functioning and sensations are a direct
reflection of your spiritual vital energy. All substances on the earth also have a
spiritual energy or pattern. These substances have been studied by homeopaths and
their patterns have been described. I will attempt to find a substance with a pattern
that is very similar to yours.

Homeopaths have found that exposing an individual to their pattern through a
similar homeopathic remedy leads to a beneficial change in their spiritual vital energy.

My education in homeopathy has come from seminar study and home study. I have
studied with the Evolution of Self School of Homeopathy (ESSH) and the Dynamis School
for a total of 340 hours of advanced homeopathic training. In addition, I have read
and studied homeopathy for the past 8 years.

Lastly, I request that you do not stop or alter any treatment that you are
receiving from any licensed healthcare professional. Any desire to change or alter
treatments should be discussed with the physician that prescribed them.
